Mehsi is located in East Champaran District of Bihar and is 40 km south of Motihari. It lies near the district’s border with Muzaffarpur District.

Patna Airport is the nearest airport. Mehsi Railway Station is on the Muzaffarpur - Motihari rail route. National Highway 28A passes through the town.
